Streamlined Referral Process for TMS & Spravato

Welcome to our referral process page for TMS and Spravato treatments. At Dr. Michael Shiekh's practice, we strive to make the referral process as seamless and straightforward as possible for both providers and patients. Below you’ll find detailed steps for referring patients to our TMS and Spravato programs, ensuring they receive the specialized care they need efficiently.


TMS Referral Process

  1. Referral Sent

    • Print, sign, and fax the TMS Clinician Referral Form to (214) 613-1667.

    • Attach a copy of the patient’s insurance card, medication history, and most recent depression measure.

  2. Initial Assessment

    • The office will contact the patient to schedule the initial assessment with Dr. Shiekh to go over treatment history and TMS eligibility.

  3. Insurance Verification and Cost Assessment

    • A TMS Care Coordinator will check insurance benefits and assess any out-of-network costs.

    • A prior authorization will be sent to insurance for coverage determination.

  4. First Treatment Appointment

    • After insurance authorization, the TMS Care Coordinator will schedule the 1-hour initial treatment appointment that will include motor threshold determination and the first TMS treatment.

  5. Begin TMS Treatment

    • The TMS Care Coordinator will schedule the remaining treatment sessions with the patient. Each session generally lasts about 30 minutes.

  6. Monitor Progress

    • We will track the patient’s response to treatment by using self-reporting depression measures and professional assessments.

Spravato Referral Process

  1. Referral Sent

    • Print, sign, and fax the Spravato Clinician Referral Form to (214) 613-1667.

    • Attach a copy of the patient’s insurance card, medication history, and most recent depression measure.

  2. Initial Assessment

    • The office will contact the patient to schedule the initial assessment with Dr. Shiekh to go over treatment history and Spravato eligibility.

  3. Insurance Verification

    • A Spravato Care Coordinator will check insurance benefits and enroll the patient in the Spravato REMS Program.

    • A prior authorization will be sent to insurance for coverage determination.

  4. REMS Pharmacy

    • After insurance authorization, the prescription will be sent to a REMS-certified pharmacy to be delivered to the office.

  5. Begin Treatment

    • Once the medication has been delivered to the office, the Spravato Care Coordinator will schedule the first eight-week treatment with the patient.

  6. Monitor Progress

    • We will track the patient’s response to treatment by using self-reporting depression measures and professional assessments.
